Terms and Conditions for Access Card Service

Introduction

Welcome to our Access Card Service. By purchasing and using our service, you agree to abide by the following terms and conditions. Please read them carefully before making a purchase.

Service Description

Our Access Card Service offers clients the opportunity to purchase a bundle of three (3) non-refundable sessions for $333. This service is intended for individuals who prefer not to use insurance.

Purchase and Use of Sessions

1. Purchase Requirements: Clients must purchase the three-session bundle upfront. Thetotal cost of the bundle is $333.

2. Non-Refundable: All purchases of the three-session bundle are non-refundable.

3. Session Validity: Clients have three (3) months from the date of purchase to use all three sessions.

4. Scheduling: Clients may choose to schedule their sessions either once a week or once a month within the three-month period.

5. Renewal: After the completion of the third session, clients will need to renew and purchase a new three-session bundle if they wish to continue using the service.

Payment

1. Payment Method: Payment for the three-session bundle must be made in full at thetime of purchase.

2. Pricing: The price for the three-session bundle is $333. This price is fixed andnon-negotiable.

Session Cancellation and Rescheduling

1. Cancellation: Sessions are non-refundable and cannot be canceled for a refund.However, clients may reschedule their sessions.

2. Rescheduling: Clients must reschedule any session at least 24 hours in advance.Failure to do so will result in the session being forfeited.

Expiration of Sessions

Three-Month Period: All sessions must be used within three months from the date of purchase. Unused sessions will expire after this period and cannot be refunded or carried over.

Client Responsibilities

1. Scheduling: It is the client’s responsibility to schedule their sessions within thethree-month period.

2. Timeliness: Clients must arrive on time for their scheduled sessions. Late arrivals mayresult in a shortened session time.

Service Provider Rights

1. Modification: We reserve the right to modify or discontinue the service at any timewithout prior notice.

2. Termination: We reserve the right to terminate the service for any client who violates these terms and conditions.

Limitation of Liability

1. No Insurance: This service is intended for individuals who choose not to use insurance.We are not responsible for any insurance claims or reimbursements.

2. No Guarantees: We do not guarantee specific results from the use of our services.
